YES — I will arrive 20 minutes before the shuttle start time to load my bike.*

YES — I understand there are no transfers, reschedules or refunds within the 24hr window of your shuttle due to weather or any other reason.*

YES — I am completing this form for myself and, if applicable, the riders listed in my reservation ("Accompanying Riders").*

YES — I am voluntarily participating in mountain biking and shuttle transportation activities.*

YES — I understand that mountain biking is an inherently dangerous activity involving significant risks of injury, paralysis, death, property damage, and other losses.*

YES — I understand that risks may arise from trail conditions, terrain, weather, wildlife, natural hazards, obstacles, road conditions, vehicle operation, other riders, spectators, and unforeseen circumstances.*

YES — I understand that shuttle transportation involves risks including vehicle accidents, loading and unloading injuries, road hazards, and transportation-related incidents.*

YES — I certify that I am physically and mentally capable of participating in mountain biking activities.*

YES — I accept full responsibility for evaluating my own skill level and determining whether any trail, feature, route, or activity is appropriate for me.*

YES — I understand that trail conditions may change without notice and may contain hazards that are marked, unmarked, known, or unknown.*

YES — I assume all risks associated with my participation, whether known or unknown, foreseeable or unforeseeable.*

YES — I accept full responsibility for my bicycle, equipment, clothing, protective gear, and personal belongings before, during, and after the shuttle service.*

I understand that mechanical failure of my bicycle or equipment may result in injury, death, or property damage.*

YES — I am responsible for inspecting and maintaining my bicycle and equipment prior to participation.*

YES — I understand that the shuttle service does not inspect, maintain, guarantee, or warrant the condition of my bicycle or equipment.*

YES — I understand that loading, transporting, securing, and unloading bicycles may result in scratches, wear, cosmetic damage, mechanical damage, or total loss.*

YES — I voluntarily assume all risks of damage to my bicycle, equipment, and personal property before, during, and after transportation.*

YES — I understand that emergency medical services may not be immediately available in remote areas.*

YES — I agree to follow all instructions, policies, rules, and safety requirements established by the shuttle operator.*

YES — I understand that failure to follow instructions or safety requirements may increase the risk of injury or death.*

YES — I release, waive, discharge, and covenant not to sue the shuttle operator, its owners, employees, contractors, drivers, agents, affiliates, volunteers, landowners, sponsors, and representatives for any claims arising from ordinary negligence related to my participation.*

YES — I agree to assume full responsibility for any injury, death, loss, damage, or expense that I may suffer arising out of or related to my participation.*

YES — I agree that this release applies to injuries, damages, losses, or claims occurring before, during, or after shuttle transportation and riding activities to the maximum extent permitted by law.*

YES — I have carefully read this agreement, fully understand its contents, understand that I am giving up substantial legal rights, and voluntarily agree to its terms.*

YES — I agree that this acknowledgment and release shall be governed by applicable state law and remain enforceable to the fullest extent permitted by law.*
